Did you know that CMS released an updated version of the ESRD Quality Reporting System (EQRS)/CROWNWeb data-collection system? This release features enhancements that are designed to support users’ data reporting needs during the next calendar year in 2020.
A key item in this update was the Medication Reconciliation (MedRec) fields on the Manage Clinical screens in CROWNWeb. CMS utilized the MedRec reporting measure for the ESRD Quality Incentive Program measure set, beginning with Payment Year 2022. The MedRec measure determines whether a facility has appropriately evaluated a patient’s medications.
Medication Reconciliation (MedRec) Reporting Measure is the percentage of patient-months for which medication reconciliation was performed and documented by an eligible professional (based on NQF #2988).
2020 Measure Technical Specifications
Number of patient-months in the denominator for which the facility reported the following required data in CROWNWeb:
- Date of the medication reconciliation
- Type of clinician who completed the medication reconciliation:
- pharmacy technician personnel
3. Name of clinician
Total number of eligible patient-months for all patients assigned to a dialysis facility during the reporting period.
Facilities with a CCN certification date on or after October 1 of the year prior to the performance period.
In-center patients who receive < 7 hemodialysis treatments in the facility during the reporting month.
The time is now, MedRec data reporting begins January 2020 for all hemodialysis and peritoneal dialysis patients. Facilities are currently not required to report MedRec data via EQRS/CROWNWeb. However, the system provides users with the ability to review the MedRec fields, in preparation for data reporting in 2020.
